STAR+PLUS Home and Community Based Services (HCBS) Waiver

Medicaid waiver that pays for long-term care services in the home or community for Texas adults age 21 and older who would otherwise need nursing home care. Services include personal attendant care, in-home nursing, respite, home modifications, adaptive equipment, home delivered meals, dental, and assisted living facility care. Family members can be hired and paid as caregivers through the Consumer Directed Services option (spouses generally cannot be paid). HCBS slots are capped, so applicants are placed on an interest list with months to years of wait time depending on region. Apply early - your time on the list does not start until you call.

Eligibility requirements

  • Texas resident age 21 or older with a disability or age 65 or older. Must meet Nursing Facility Level of Care (NFLOC) standard. Income at or below 300% of the SSI Federal Benefit Rate ($2,982 per month in 2026 individual, $5,964 couple). Countable assets at or below $2,000 individual ($3,000 couple). Primary home generally exempt up to $752,000 in equity. Excess income can be redirected through a Qualified Income Trust (Miller Trust).
