Texas Medicaid Buy-In Program (MBI)

Administered by Texas Health and Human Services Commission (HHSC) · hhs.texas.gov

Lets working Texans with disabilities earn substantially more than standard Medicaid limits and still keep full Medicaid coverage by paying a sliding scale monthly premium. Designed to remove the financial penalty that traditionally forced disabled adults to choose between working and keeping their health coverage. Covers all standard Medicaid benefits. Premiums are calculated using a formula based on countable income. Maximum premium is $500 per month combined.

Eligibility requirements

  • Texas residents ages 16 to 64 with a qualifying disability who are working (W-2 wages or self-employment with FICA tax paid). Earned income up to 250% FPL ($3,325 per month for an individual in 2026). Countable resources at or below $2,000. Sliding scale monthly premium based on income. Apply by calling 1-800-252-8263.
